If you wish to apply for rematriculation, please read the below instructions carefully and follow the steps one by one. Please contact the Academic Affairs Registrar’s Office ([email protected] or 032-626-1115) directly if you have any questions while filling out the form.
- Please submit the Google Form: https://forms.gle/ddU9kY8av2xNY5PJ9
- If you extend your leave after military leave, your graduation term will be reverted to your returning semester even if you submit the certificate of military service.
- You must submit the Certificate of Military or Certificate of Career (in English) to maintain your original requirement term. Documents must include your official discharge date.
- Book an appointment with your coordinator here. This meeting is mandatory.
- Pay $50.00 on “Solar” after a few days of your meeting with the CS coordinator.
- Solar > Account Information/Payment > What Do I Owe?
- A non-refundable $50.00 rematriculation fee will be posted to your student account at the time of processing this request
